Higgins looked dead and buried when he trailed 8-3 and then 9-4. But the four-time champion refused to give up. He cut the deficit to 9-7 overnight, then turned on the style in the final session.
Higgins fired three century breaks in the deciding frames to complete his remarkable recovery. His aggressive play frustrated O'Sullivan, who was chasing a record-tying eighth world title.
The drama didn't stop there. World number one Judd Trump also crashed out on the same day, losing 13-12 to Iranian qualifier Hossein Vafaei. Vafaei reached the quarterfinals for the first time in his career.
The double elimination removed two of the tournament's biggest names in one shocking day. Higgins now advances to face either Neil Robertson or Ricky Walden in the quarterfinals.
This was one of the biggest upsets in snooker history. O'Sullivan was chasing his eighth world title, which would have tied the all-time record. The loss of both O'Sullivan and the world's top player Trump in one day shocked the sport.
Higgins will play his quarterfinal match later this week. The World Championship continues through early May.
